Vision & Dental Insurance

Dental insurance includes:

  • No waiting period
  • Ability to visit any dentist nationwide
  • Annual maximum benefit of $1,500 per-person
  • Preventative and Diagnostic services covered at 100%
  • Coverage for fillings, root canals, and oral surgery

Vision insurance includes:

  • No waiting period
  • Ability to visit any provider nationwide
  • $10 copay for annual eye exam
  • $25 copay for standard prescription lenses
  • $80 allowance for contact lenses
  • $100 allowance for frames

There are two enrollment periods:

  • Fall Open Enrollment: July 1-September 30 (Plan effective date is August 1st - 12 and 6-month plans available)
  • Spring Open Enrollment: January 1 – March 31 (Plan effective dates is February 1st – 6-month plans only)
