High School Visits to Malpass Library

Please Note: This form should be submitted at least two weeks in advance of the visit. See the Policy on Visits to the Library by Area High Schools (Adobe Acrobat format).

Requests submitted with less than two weeks notice may not be filled. Please remember that visits are not permitted during the final two weeks of any WIU semester. If students intend to check out books, the school must send in advance a list of names by fax (309-298-2791) or surface mail to:
