Fitness Assessment Request Form
The purpose of a Fitness Assessment is to let you know where you rank in your age group for overall health and fitness. A personal trainer will be able to let you know where you rank by performing any of the following possible assessments: a three-minute step test, resting heart rate, body fat, height and weight, circumference measurements, push-up test, curl-up test, sit and reach, and bench press.
Simply fill out this form and a Campus Recreation staff member will contact you to set up a time for a personal fitness assessment.
