This form is to be completed by those international students who have applied to WIU within the last two years but did not attend. If you applied more than two years ago, you should complete the application for new students.

Email clear scanned copies of missing admission documents to  International-ed@wiu.edu. Original hard copy documents to complete conditions of admission should be mailed to:
